PERSONAL INJURY GLOSSARY

Treatment on Lien

A care arrangement where a provider treats a personal injury client without upfront payment, securing a lien against the eventual settlement to recover the cost.

Treatment on lien is the umbrella term for any medical, chiropractic, imaging, or pharmacy care delivered to a personal injury client without upfront payment, secured by a lien against the eventual settlement. The provider absorbs the timing risk in exchange for an enforceable claim on the recovery.

Treatment on lien matters because the alternative is a client who cannot afford care, falls out of the treatment plan, and ends up with a thin medical record. Adjusters and defense counsel weaponize treatment gaps. A clean, continuous medical record materially affects settlement value. Treatment on lien is what keeps that record continuous when the client lacks insurance, lacks cash, or both.

Pharmacy treatment on lien is a specific subset: prescription medications dispensed on credit, with the cost secured by a pharmacy lien against the settlement. CreoRx's lien pharmacy serves exactly this need. The client gets prescriptions at any of 67,000+ pharmacies with no upfront cost; the firm has portal access to every fill; the lien resolves at settlement.

Treatment on lien arrangements are routine in personal injury practice but operationally heavy. Each provider has its own paperwork, its own lien letter format, its own reduction practice. Standardizing as much as possible (master agreements, single intake, single portal) reduces case-manager workload and tightens the lien resolution timeline.
